Position Overview
A&E Mobile Healthcare is seeking a compassionate, resilient, and proactive Peer Support Specialist to join our mobile harm reduction and healthcare team. In this role, you will engage individuals in the community by providing outreach, case management support, harm reduction education, and connections to healthcare and social services. This position is ideal for individuals with lived or professional experience supporting people who use drugs (PWUD) and a strong commitment to person-centered, non-stigmatizing care.
Key Responsibilities
- Conduct community outreach and engage individuals experiencing barriers to healthcare.
- Provide case management support and connect participants with medical, behavioral health, housing, and social services.
- Advocate for participants by addressing Social Determinants of Health (SDOH) through community resources and partnerships.
- Deliver harm reduction education, distribute harm reduction supplies, and provide overdose prevention education, including Naloxone training.
- Accurately document participant interactions and services using Electronic Health Records (EHR) and Microsoft Office applications.
Qualifications & Requirements
- Lived or professional experience working with people who use drugs (PWUD) or related populations.
- Strong communication skills using empowering, culturally competent, and non-stigmatizing language.
- Ability to work effectively in fast-paced community environments with minimal supervision.
- Basic proficiency with Microsoft Office and the ability to learn Electronic Health Record (EHR) systems.
- High school diploma or associate degree preferred, but not required. Relevant lived experience and community expertise are highly valued.

Community Health & Harm Reduction Specialist
Position Type: Full-Time (Occasional Saturday Availability Required)
Location: Baltimore, MD (Baltimore City residents only)
Reports To: Program Manager / Clinical Director
Position Overview
The Community Health & Harm Reduction Specialist plays a vital role in connecting underserved individuals with medical, behavioral health, and social services. This position focuses on community outreach, case management, participant engagement, and care coordination while promoting harm reduction principles and improving access to healthcare throughout Baltimore City.
Key Responsibilities
- Conduct street-level outreach and community engagement to connect individuals with available healthcare and harm reduction services.
- Provide case management and care coordination by helping participants navigate healthcare systems and access essential resources.
- Utilize motivational interviewing techniques to support participant-centered care and behavior change.
- Maintain accurate documentation of participant interactions within the Electronic Medical Record (EMR) system.
- Collaborate with healthcare providers, community organizations, and multidisciplinary teams to improve participant outcomes.
Qualifications & Requirements
- High school diploma or GED required; an Associate’s or Bachelor’s degree in Public Health, Social Work, Human Services, or a related field is preferred.
- At least one year of experience working with people who use drugs (PWUD), individuals experiencing homelessness, or vulnerable populations.
- Must currently reside in Baltimore City and have strong knowledge of local neighborhoods and community resources.
- Familiarity with Social Determinants of Health (SDOH) and multidisciplinary care coordination.
- Basic technical proficiency and willingness to learn Electronic Medical Record (EMR) systems.
- Commitment to harm reduction, trauma-informed care, and cultural humility.
- Willingness to work occasional Saturdays for outreach events and community activities.
